Description

American Traveler is hiring an RN Case Manager for a hospital Care Management department, requiring 2 years of case management experience and an active NC or compact RN license.

Details

• Hospital-based Care Management unit serving all units of a 815-bed facility
• Patient ratio of 1:40
• Uses Cerner, NATE, and EnsoCare (post-acute) documentation systems
• Self-scheduling required
• Floating required to units as needed
• Day shift schedule from 8:00 AM to 4:30 PM

Requirements

• Active RN license (NC or Compact) currently in hand
• Minimum 2 years of recent case management experience required
• Strong documentation and communication skills required
• Ability to shift attention between multiple tasks
• Ability to build trust with team, patients, and families
• US Social Security Number and date of birth required for consideration (no Canadian SSN accepted)
• One professional reference required (two required for Florida and Missouri candidates)
• No gaps in work history of 90+ days within the past 12 months
