    Responsibilities
    - Provide excellent patient care by assisting in the collaboration, development, implementation, revision and reporting of the case management program.
    - Act as a liaison between the patient, family, nurse, physicians, multidisciplinary team and patient’s healthcare benefactor to optimize outcomes.
    - Serve as a consultant to the healthcare team on specific patient items.
    - Coordinate a continuum of care for a defined population of patients from pre-admission through post discharge.
    - Assure that patients and families proceed efficiently through the course of hospitalization.

    Requirements
    - Active RN license in good standing (state-specific as required).
    - 1+ year of RN experience in case management or care coordination.
    - Strong communication, advocacy, and collaboration skills.
    - Ability to manage patient care across the continuum from admission to discharge.
    - BSN preferred; CCM or case management certification preferred.
    - Ability to work in a travel assignment environment and adapt to diverse care settings.
