Description

American Traveler is seeking a Home Health Physical Therapist with at least 1 year of home health experience and 2 years overall clinical experience to serve patients in the Sonoma/Marin area.

Details

• Home health setting covering geriatric patients requiring wound care and other home health services
• Day shift schedule, 5x8 hours, 8:00 AM to 5:00 PM
• No weekend shifts required
• Minimum 23-24 visits per week required to qualify for a 40-hour guarantee
• On-call is discretionary and not required
• May be asked to cover surrounding counties in addition to primary territory
• Charting completed using Kinnser/Wellsky and must be done on-site in the clinic, not taken home
• Mileage reimbursed at IRS standard rate from first patient to last patient

Requirements

• Minimum 1 year of home health experience and 2 years overall clinical experience required
• Current BLS certification required
• Current state license required at time of consideration, with no license hits; license required if applicable
• Current driver's license required
• Must have reliable transportation and insurance
• OASIS experience preferred
• Full 7-year work history required with any gaps explained
• Two professional references from a supervisor within the past 2 years required
