Key Responsibilities
- Provide skilled physical therapy assistant services to patients in a home health setting
- Manage a geriatric-focused caseload, visiting up to 6 patients per day
- Maintain productivity standards of 30 productivity points per standard workday
- Document patient care accurately and in a timely manner using HCHB electronic medical records
- Travel independently to patient homes within the service area, with occasional visits extending beyond a 50-mile radius
Required Qualifications
- Active California Physical Therapy Assistant (PTA) license
- Minimum of 1 year of recent skilled home health experience (at least 1 year within the last 3 years)
- Proficiency with HCHB electronic medical records system
- Current Basic Cardiac Life Support (BCLS) certification
- Reliable personal transportation for patient home visits
Preferred Experience / Skills
- Experience working with geriatric patient populations in a home health environment
- Familiarity with efficient caseload and route management in a field-based setting
