Key Responsibilities
Conduct comprehensive patient evaluations and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
Perform 60-minute initial evaluations and 30-minute follow-up visits in an outpatient setting
Complete medical record documentation in a clear, concise, accurate, and timely manner including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
Design functional treatment programs focused on progressing patients toward discharge and effective return to community activities
Delegate and direct discipline-specific therapy services for assigned patients
Serve as an educator to patients, families, students, and coworkers; participate in student supervision programs as assigned
Required Qualifications
Active and unrestricted Physical Therapist licensure in good standing in the state of Colorado
Adherence to state-specific Physical Therapy Practice Act and all applicable regulatory agency requirements
Ability to complete all state-mandated continuing education requirements
Demonstrated competency in clinical reasoning, treatment planning, and documentation standards
Commitment to patient and employee safety practices including safe patient handling and infection control protocols
Preferred Experience / Skills
Prior experience in an outpatient physical therapy setting
Experience supervising or mentoring physical therapy students
Involvement in professional development activities, continuing education, or professional organizations
