Key Responsibilities
Conduct 60-minute initial evaluations and 30-minute follow-up sessions for assigned outpatient caseload
Develop, implement, and modify individualized treatment plans aligned with patient needs, goals, and physician direction
Complete thorough medical record documentation including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
Delegate and direct discipline-specific therapy services for assigned patients, ensuring functional treatment programs support return to community activities
Educate patients, families, students, and coworkers as part of a comprehensive care approach
Adhere to the state Physical Therapy Practice Act, maintain licensure in good standing, and fulfill all continuing education requirements
Required Qualifications
Active Physical Therapist license in good standing in the state of Colorado
Completion of all state-mandated continuing education requirements
Ability to perform comprehensive patient evaluations and develop evidence-based treatment plans
Compliance with applicable professional codes of conduct and ethical standards
Preferred Experience / Skills
Prior experience in an outpatient therapy setting
Experience supervising or mentoring PT students in a clinical environment
