Key Responsibilities
Complete comprehensive evaluations of assigned patients and develop, implement, and modify individualized treatment plans in alignment with patient needs, goals, and physician direction
Perform 60-minute evaluations and treatments, progressing patients toward discharge within an established length of stay
Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under signed, valid physician orders and within insurance approvals
Serve as an educator to patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations
Adhere to discipline-specific State Practice Act and regulatory requirements; maintain professional licensure in good standing and fulfill all state-mandated continuing education requirements
Required Qualifications
Active Physical Therapist license in good standing in the state of Colorado
Ability to complete comprehensive patient evaluations and develop evidence-based treatment plans
Proficiency in medical record documentation in compliance with state, federal, and payer guidelines
Commitment to patient and employee safety standards and safe patient handling practices
Preferred Experience / Skills
Prior experience in an outpatient physical therapy setting
Experience supervising or mentoring physical therapy students
Active involvement in continuing education and professional development organizations
