Key Responsibilities

• Evaluate and treat patients in a skilled nursing facility setting to restore function and improve quality of life
• Develop and implement individualized physical therapy treatment plans based on patient needs and goals
• Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
• Educate patients and families on therapeutic exercises, safety, and home programs

Required Qualifications

• Active Physical Therapist (PT) license in the state of Washington
• Graduate of an accredited Physical Therapy program
• Ability to work a Monday-Friday daytime schedule in an onsite SNF environment

Preferred Experience / Skills

• Prior experience working in a skilled nursing facility or long-term care setting
• Familiarity with a high-census, 100-bed facility environment
• Strong documentation and communication skills
