Key Responsibilities
- Provide physical therapy services to patients in a home health setting across an assigned service territory
- Meet a productivity expectation of a minimum of 30 points per week
- Complete all clinical documentation by 10:00 AM the following day
- Utilize HCHB (Homecare Homebase) for documentation and care coordination
- Travel throughout the assigned service area to conduct patient visits
Required Qualifications
- Active Physical Therapist (PT) license in the state of Washington
- Minimum of one (1) year of home health experience required
- Proficiency with HCHB (Homecare Homebase) EMR system
- Must reside at least 50 miles from the facility to be eligible
- Reliable transportation and ability to travel throughout the assigned service territory
Preferred Experience / Skills
- Familiarity with the greater Snohomish County area and surrounding communities
- Strong time management and self-directed work habits suited for a home health environment
- Experience managing a high-volume caseload in a home health setting
