Description

American Traveler is seeking a Physical Therapist for a Home Health float position requiring 1+ year of home care experience and an active MA PT license.

Details

Home health setting providing physical therapy services to patients in their place of residence
Serves the South Region including Dorchester, Jamaica Plain, Mission Hill, and Roxbury
Patient population ranges from infants to adults over 100 years old
Standard shift is 8:00 AM-4:30 PM
Weekend rotations occur once every 6 weeks
Must be able to travel to patient homes as assigned
Uses Epic for charting

Requirements

Active Massachusetts Physical Therapist license required (license required if applicable)
Minimum 1 year of home care experience required
Must be comfortable with electronic documentation in accordance with agency policies and standards of practice
Strong interdisciplinary communication and collaboration skills required

Additional Information

Collaborate with patients, families, physicians, and other allied health professionals to identify needs and coordinate care
Provide timely and thorough documentation in accordance with home care policies and procedures
Flu vaccine required during flu season with no religious or medical exemptions allowed
Local candidates must reside at least 60 miles from Boston to qualify for standard rate
