Description

American Traveler is seeking a Home Health Physical Therapist for a float position requiring an active MA PT license and 1+ year of home care experience.

Details

• Home health setting providing physical therapy services to patients in their place of residence
• Patients range in age from birth to over 100 years with physical impairments, functional limitations, or disabilities
• Covers North Region locations including Beverly, Marblehead, Salem, and Swampscott
• Requires ability to travel to assigned cases across the coverage area
• Shift schedule 08:00-16:30
• Weekend rotation once every 6 weeks
• Uses Epic for electronic documentation

Requirements

• Active Physical Therapist license in Massachusetts required; license required if applicable
• Minimum 1 year of home care experience required
• AHA certifications only
• Flu vaccine required with no religious or medical exemptions allowed
• Must be able to collaborate with patients, families, physicians, and other allied health professionals

Additional Information

• Responsibilities include evaluating and treating patients and coordinating care with interdisciplinary teams
• Utilizes timely electronic documentation in accordance with agency policies and standards of practice
