Job Details

- Responsibilities: Conduct regular physical therapy visits, complete documentation within 1 day of patient visits, utilize the EMR system HomeCare HomeBase (HCHB)
- Certifications: BLS Certification required
- Additional Information: Must have prior home health experience unless otherwise specified; EMR system is HomeCare HomeBase; manager/supervisor is available 24/7; shifts are 8 hours in daytime; cancellation policy allows up to 4 shifts cancellation per 13-week assignment; charting must be completed promptly; license submission is required; trauma level and care setting are specified as home health.
