Job Summary
Physical Therapist (PT) specializing in Home Health (HH) services in the designated area. This role involves providing outpatient rehabilitation to geriatric patients within a community health setting. The position requires flexibility with territory coverage, timely documentation, and adherence to clinical documentation standards. The assignment begins on October 5, 2026, and concludes on January 4, 2027.

Job Details

  • Schedule: Days shift, 8 hours per day, minimum of 40 hours per week.
  • Required Qualifications: Valid license in hand with active drivers license and personal transportation, minimum of 2 years of relevant experience including home health with OASIS and proficiency in Homecare Homebase (HCHB). No first-time travelers accepted.
  • Preferred Qualifications: Experience with outpatient rehabilitation, geriatric patient care, and outpatient therapy modalities.
  • Additional Information:
    • Must have at least 1 year of relevant experience and OASIS experience for staff completing Start of Care.
    • RTO (Requested Time Off) must be submitted at the time of application.
    • On-call rotation may be requireddiscuss during interview.
    • Modules are billable and completed during orientation.
    • Home health and hospice experience preferred but not mandatory.
    • Mobile and flexible to cover up to a 60-mile territory radius from the hub/office.
    • Timely communication and documentation are mandatory for success in this role.
    • All travelers are required to have an active drivers license and personal transportation.
    • Start date scheduled for 10/05/2026; end date 01/04/2027.
