Key Responsibilities

  • Conduct comprehensive patient evaluations (60-minute eval + treatment) and develop, implement, and modify individualized treatment plans in accordance with patient needs, goals, and physician direction
  • Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and payer guidelines
  • Design functional treatment programs focused on progressing patients toward discharge within an established length of stay and facilitating effective return to community activities
  • Delegate and direct discipline-specific therapy services for assigned patients, ensuring care is provided under valid physician orders and within insurance approvals
  • Serve as an educator for patients, families, students, and coworkers; participate in student programs by providing supervision, developing learning objectives, and completing student evaluations as assigned
  • Adhere to all safety standards including safe patient handling, infection control, and workplace hazard reporting as part of an organization-wide safety culture

Required Qualifications

  • Active and unrestricted Physical Therapist license in the state of Ohio
  • Compliance with all state-mandated continuing education requirements
  • Adherence to the discipline-specific State Practice Act and applicable professional codes of conduct
  • Ability to complete thorough clinical documentation in compliance with state, federal, and intermediary guidelines

Preferred Experience / Skills

  • Prior experience in an outpatient physical therapy setting
  • Experience with student supervision and clinical education programs
  • Demonstrated commitment to ongoing professional development through continuing education and professional organization involvement
