Contract | Full-Time | Onsite | Days, Monday–Friday 8:00 AM–5:00 PM (5x8-hour shifts, 40 hours/week)
Key Responsibilities
- Serve as a clinical resource for outpatient and inpatient clinicians regarding HIV and/or Hepatitis C pharmacotherapy.
- Collaborate with the Inpatient Infectious Diseases Clinical Pharmacist to transition HIV patients from the hospital setting to the ambulatory HIV Clinic on appropriate therapy.
- Develop and optimize pharmacotherapeutic regimens through collaborative practice agreements, including drug utilization reviews and monitoring for drug interactions and potential adverse events.
- Work to minimize polypharmacy and develop individualized patient medication action plans with self-management goals, seeing approximately 14 patients per day as a mid-level provider.
- Precept pharmacy students and residents, and participate in didactic and experiential education activities.
- Function as a key member of the core multidisciplinary team within the Medical Home model of patient care.
Required Qualifications
- Doctorate of Pharmacy (PharmD) or equivalent degree from an accredited College of Pharmacy.
- Completion of a 2-year ASHP (American Society of Health-System Pharmacists) residency with a focus in HIV.
- Minimum of 5 years of direct HIV patient care demonstrating medication management expertise.
- Active Pharmacy license issued by the Texas State Board of Pharmacy, or eligibility to obtain licensure.
- Current Basic Life Support (BLS) certification required.
Preferred Experience / Skills
- Experience managing Hepatitis C pharmacotherapy in an ambulatory or outpatient setting.
- Prior experience in a multidisciplinary or Medical Home care team environment.
- Background in precepting pharmacy students or post-graduate pharmacy residents.
