Job Summary
Outpatient Physical Therapist (PT OP) Location: Not specified. This outpatient therapy role involves evaluating and treating diverse patients in a clinical setting. The position requires developing individualized treatment plans to support patients' return to community activities, along with providing education to patients, families, and coworkers.
Job Details
- Position Description: Evaluates assigned patients, develops, implements, and modifies treatment plans based on patient needs and physician directives. Responsible for delegation and management of therapy services for designated patients, ensuring functional treatment programs.
- Completes comprehensive evaluations, utilizing clinical reasoning to establish goals aimed at progressing patients toward discharge within an established length of stay.
- Performs medical record documentation accurately, concisely, and in a timely manner, including goal writing, functional assessments, letters of medical necessity, and charge entry, following all regulatory guidelines.
- Provides care under signed, valid physician orders and as insurance approvals permit.
- Participates actively in student programs by providing supervision, development of learning objectives, and student evaluations.
- Creates and pursues a professional development plan, engaging in continuing education, self-study, and professional activities.
- Adheres to State Practice Act and other regulatory standards; maintains licensure in good standing and completes mandated continuing education.
- Demonstrates understanding of organizational performance improvement initiatives and offers constructive feedback for quality enhancement.
- Required Qualifications: Degree in Physical Therapy from an accredited school; 6 months to 1+ year of outpatient clinical experience; CPR certification; active and applicable state license; ability to work an 8-hour shift; minimum of 40 hours per week.
- Schedule: Days shift, 8 hours per day, minimum 40 hours per week.
- Additional Information: The role involves completing comprehensive patient evaluations, developing and adjusting treatment plans, documenting accurately, participating in professional development, and adhering to safety and regulatory standards.
