  • Position Description: Performs patient evaluations, develops individualized treatment plans, and modifies care based on patient needs and physician directions. Responsible for delegating therapy services and designing functional treatment programs. Acts as an educator and participant in clinical and organizational development activities.
  • Certifications, Education, and Experience Requirements: Degree in Physical Therapy from an Accredited School; 6 months to 1+ Year of Outpatient Clinical Experience; CPR Certification; Active applicable license in the state of practice; 1 year of experience preferred, with a 6-month duration desired.
  • Schedule: Day shift, 8-hour shifts, minimum 40 hours per week.
  • Additional Job Responsibilities: Completes comprehensive patient evaluations, documents in medical records accurately and timely, ensures care aligns with physician orders and insurance requirements, and participates in student training and professional development activities. Adheres to regulatory and organizational standards, promotes safety, and engages in performance improvement initiatives.
  • Preferred Qualifications: Experience with previous agency assignments and prior work with the organization are highly desired; experience with outpatient settings is preferred.
