Key Responsibilities

• Evaluate patients and develop individualized occupational therapy treatment plans within a skilled nursing facility setting
• Provide therapeutic interventions to improve patients' functional independence in daily living activities
• Monitor and document patient progress, adjusting treatment plans as clinically appropriate
• Collaborate with interdisciplinary care teams to ensure coordinated patient care
• Maintain accurate and timely clinical documentation in accordance with facility and regulatory standards

Required Qualifications

• Active Occupational Therapist (OT) license in the state of Hawaii or eligibility to obtain one
• Graduate of an accredited Occupational Therapy program
• Experience working in a skilled nursing facility or similar post-acute care environment
• Strong clinical documentation and communication skills

Preferred Experience / Skills

• Prior travel or contract therapy experience
• Familiarity with MDS and skilled nursing facility regulatory requirements
• Experience with a variety of patient populations including geriatric care
