Provides rehabilitative, skilled, and medically necessary treatment interventions to patients. Administers various procedures as part of the rehabilitation plan, including ADL retraining, therapeutic activities, therapeutic exercises, and use of supportive and assistive devices. Assesses patient progress and adjusts treatments accordingly in the clinical record. Evaluates treatment outcomes for effectiveness and modifies services as needed based on evaluations. Provides consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders. Effectively communicates with supervisors and health team members regarding patient progress, barriers, and treatment plans. Provides comprehensive discharge summaries of services provided. Documents all required information in the electronic documentation system and maintains patient records. Accurately records treatment minutes for services provided. Participates in required meetings, including patient care conferences and rehabilitation meetings. Participates in facility and company-required trainings and in-services. Able to obtain necessary durable medical equipment or supplies per regulatory and clinical practice requirements. Able to supervise and support occupational therapist assistants and rehabilitation aides. Adheres to professional practice acts, code of conduct, and ethics during practice as an Occupational Therapist.
