Absolute Minimum Qualifications & Departmental Hardstops:

  • Licensure Mandate: Active, unencumbered New York State (NYS) RN License held in hand at submission.
  • Core Certification Required: Active AHA BLS certification.
  • Role Versatility Mandate: Candidate must agree to drop down to cover an LPN Medication Cart position when operational staffing permits/requires.
  • Physical & Clinical Capabilities: Ability to lift up to 50 lbs, stand/walk for extended durations, and operate facility-specific medical equipment.
  • Leadership Experience Baseline: Prior RN Team Lead, Charge Nurse, or Supervisory experience within long-term care / skilled nursing facilities.

Job Summary & Clinical Scope

A Nursing Facility in Montour Falls, NY is seeking an experienced, flexible Skilled Nursing Registered Nurse Team Lead (RN SNF) for a 13-week evening contract in SNF 3 (Department 3005). Acting as the primary clinical shift lead, the RN will direct nursing workflows across a 40-resident unit, supervise interdisciplinary floor staff, advocate for resident safety, and manage acute medical status changes.

  • Clinical Team Leadership & Delegation: Oversee evening shift operations for a 40-resident unit, coordinating care alongside 2 LPNs and 3–4 CNAs under the direction of the RN Unit Manager.
  • Resident Assessment & Status Changes: Conduct comprehensive assessments, monitor chronic/acute conditions, record physiological changes, and report urgent clinical deviations to attending physicians and family members.
  • Medication Administration & Cart Coverage: Administer prescribed medications and treatments; step into direct medication cart coverage (LPN role execution) as needed to support unit staffing balances.
  • Interdisciplinary EMR Documentation: Document daily progress notes, incident reports, and care plan updates thoroughly within the electronic health record system while ensuring compliance with NYS Department of Health SNF regulations.
