The role involves medication review and patient education. The nurse will review after-visit summaries and manage the EPIC in-basket. Support will be provided to the Certified Medical Assistant (CMA) with clinical concerns. Coordination of care and clinical management of patients and caregivers is required. This includes handling prescription refills, managing clinics, and triaging calls. Medication reconciliation and reviewing charts are also part of the duties. Collaboration with other disciplines such as pharmacy, social work, and nutrition is expected. This position operates Monday through Friday from 8:00 AM to 4:30 PM. There are no weekend, call, or holiday shifts associated with this role.
