Key Responsibilities of the Home Health RN Case Manager

• Perform comprehensive assessments of high-risk members
• Collaborate with primary care providers to implement individualized care plans
• Address and reassess cost-efficient levels of care
• Coordinate non-medical support for treatment plan compliance
• Manage care transitions with healthcare team collaboration
• Maintain detailed clinical, functional, and fiscal records

Requirements of the Home Health RN Case Manager

• Associate or Bachelor's Degree in Nursing or related field
• Current, unrestricted State Registered Nurse license in Maryland
• CPR/BLS certification
• Valid driver's license and car insurance
• Wound care experience
• Proficiency in Point of Care/HomeCare HomeBase and OASIS
• Home health experience
• Case Management certification preferred
