The RN provides care to these patients using the nursing process, nursing trends, technology, and available resources. Performs all responsibilities and duties required by the unit specific to the scope of service. Implements procedures and theories related to the specific area of practice. Maintains up-to-date and accurate documentation of nursing care provided. Completes initial assessment upon patient admission and develops an appropriate plan of care. Implements the plan of care, nursing interventions, and patient care procedures. Modifies the plan of care based on continuous evaluation. Takes responsibility for patient and employee safety by reporting and/or correcting safety hazards and incidents. Communicates directly with others to resolve issues when necessary. Offers constructive feedback to assist in the professional development of peers. Confers with colleagues to expand knowledge base. Identifies personal learning needs and takes initiative to find appropriate solutions. Fulfills all requirements of departmental competencies. Receives patient bedside shift report from an RN at the beginning of each shift. Ensures visual validation of a safe patient room environment at the time of shift change. Updates the patient communication board in each patient room. Delegates patient care tasks to assigned team members and validates task completion throughout the shift. Completes and documents basic RN patient care activities such as assessments and vital signs interpretation. Ensures physician order completion and documents admission/discharge of patients. Completes hourly rounding on each assigned patient and updates care plan documentation. Provides patient medications using the five rights: Right Patient, Right Medication, Right Route, Right Dose, and Right Time. Maintains patient safety as the highest priority. Ensures compliance with safety measures such as fall precautions and pressure ulcer prevention. Maintains a positive attitude that promotes a healthy work environment. Focuses on prompt answering of patient call lights to ensure effective triage and completion of patient requests. Uses critical thinking to evaluate the overall patient condition and nursing judgment to identify changes in patient status. Effectively communicates within the healthcare team to promote collaboration and patient safety. Manages time efficiently to enable quick admission and discharge processes for patients.
