Home Health RN Job Details

• Weekly Estimated Pay: $1,830–$1,930
• Assignment Type: 13-week travel nursing assignment
• Location: Williamsville, NY, serving patients throughout Erie County
• Start Date: October 12, 2026
• End Date: January 11, 2027
• Schedule: Primarily Monday–Friday, 8:00 a.m.–5:00 p.m.
• Shifts per Week: 5
• Shift Duration: 8-hour days
• Weekend Coverage: Approximately one Saturday per month, 8:00 a.m.–5:00 p.m.; candidates must also be open to applicable on-call rotations
• Service Area: Erie County, with travel of up to 75 miles from the assigned care center and approximately 250 miles per week
• Patient Volume: Approximately 5–8 patients per day, with assignments potentially ranging from 5–10 patients
• Productivity: Expectation of approximately 30 productivity points per week
• Hours: Contract hours are not guaranteed; additional nearby coverage opportunities may be available when patient census is low

Home Health Registered Nurse Requirements

• Current Registered Nurse (RN) license in hand; pending licenses are not accepted
• At least 1 year of recent adult home health nursing experience required; hospice experience may be considered depending on specialty
• Recent home health experience required; first-time home health clinicians are not accepted
• Current CPR/BLS certification
• Recent proficiency with HomeCare HomeBase (HCHB), preferably used within the past 6 months
• Proficiency with OASIS, preferably used within the past 6 months; retraining may be required if experience is older
• Valid driver’s license, reliable personal transportation, and current auto insurance
• Ability to travel between patient homes and attend periodic office meetings
• Comfort with completing all clinical documentation within 24 hours of each patient visit
• Willingness to accept scheduled patients throughout the assigned territory, including longer drives between visits

Home Health RN Responsibilities

• Provide skilled nursing care in patients’ homes according to physician orders, care plans, and established clinical standards
• Perform comprehensive assessments, start-of-care visits, resumption-of-care visits, recertifications, routine visits, transfers, and discharges
• Manage a variety of home health needs, including wound care, medication management, ostomy and feeding tube care, catheter care, IV therapy, pain management, oxygen administration, and symptom monitoring
• Complete OASIS assessments and accurate, timely clinical documentation in HCHB
• Monitor patient progress, identify significant changes in condition, and communicate updates to the care team and providers
• Coordinate services with patients, families, physicians, therapists, aides, and other members of the interdisciplinary team
• Provide patient and caregiver education focused on medications, disease management, safety, infection prevention, and plan-of-care goals
• Directly supervise and evaluate home health aides and LPN/LVNs as assigned
• Travel safely and efficiently throughout the assigned territory while maintaining a high standard of compassionate, patient-centered care
