Travel RN – Hospice

- Admit patients to hospice services and complete comprehensive assessments
- Assess physical, psychosocial, environmental, safety, and developmental needs
- Develop and implement individualized plans of care
- Document patient assessments, interventions, and care provided
- Coordinate care with the multidisciplinary hospice team
- Conduct follow-up visits based on patient needs
- Participate in hospice Quality Assessment & Performance Improvement (QAPI) initiatives
