Hospice Registered Nurse (RN)

Job Summary

Join a dedicated hospice team as a Registered Nurse, providing compassionate and comprehensive care to patients and their families in Sunnyvale, California. This role involves managing a caseload across San Francisco, San Mateo, and Santa Clara Counties, focusing on holistic patient assessment and individualized care planning.

The Hospice RN is responsible for admitting new patients, conducting thorough physical, psychosocial, environmental, safety, and developmental assessments. You will develop and implement tailored plans of care, meticulously document observations and interventions, and ensure seamless coordination with a multidisciplinary team to support patients through their end-of-life journey.

This is a 13-week contract assignment, offering an opportunity to make a significant impact within a quality-focused hospice program.

Key Responsibilities
  • Admit patients to hospice services, initiating the care process with empathy and professionalism.
  • Conduct comprehensive assessments of patients' and families' physical, psychosocial, environmental, safety, and developmental needs.
  • Develop, implement, and continuously evaluate individualized plans of care based on assessment findings.
  • Document all patient observations, interventions, and care plan modifications accurately and timely.
  • Coordinate and collaborate effectively with a multidisciplinary team to ensure integrated patient care.
  • Participate actively in the hospice program's quality assessment performance improvement (QAPI) initiatives.
  • Perform follow-up visits as needed to monitor patient progress and adjust care plans accordingly.
Required Qualifications
  • Active Registered Nurse (RN) license.
  • Minimum of one (1) year of recent Hospice experience.
