Job Title: RN Case Manager Profession: Nursing Specialty: Hospice RN Duration: 8 weeks Shift: Monday-Friday Hours per Shift: 8 hours Experience: Minimum 2 years License: Current RN license in the State of Montana required Certifications: BSN preferred, valid driver's license, appropriate auto insurance, and reliable transportation required Must-Have: 2 years of recent acute care, home care, or hospice experience, including clinical skills such as venipuncture, catheterization, wound care, and parenteral therapy preferred. HCHB experience is strongly preferred. Description: The RN Case Manager provides skilled nursing, care, and case management for assigned patients in their place of residence. The RN admits patients onto services and coordinates and integrates services to ensure continuity of care. The RN facilitates the achievement of quality, clinical, and cost outcomes. The RN negotiates, procures, and coordinates services and resources needed by the patient or family. The RN creates opportunities and systems to enhance outcomes. The RN uses the nursing process of assessment, planning, and implementation of care. The RN provides patient and family instruction and evaluation through an interdisciplinary team approach. This position may start as a Visiting RN and move to Case Manager. The same clinical responsibilities apply to both designations. The RN provides skilled nursing and coordinates care for patients and family members in a residential setting through a team approach. The RN observes, assesses, evaluates, and plans interventions in relation to patients' physical and psychosocial statuses. The RN monitors changes in condition and/or reactions to treatments or medications throughout contact with patients. The RN oversees care provided by Home Health Aides and CNAs to assigned patients and evaluates compliance with the plan of care. The RN assures continuity of care through effective communication and information exchange with other disciplines, community health, support, and social agencies. The RN provides quality care and appropriate symptom management for positive clinical outcomes. The RN promotes optimal health and quality of life through teaching, counseling, appropriate interventions, and referrals. The RN maintains accurate, current, and complete patient records in accordance with agency standards and OASIS requirements. The RN participates in team scheduling coordination and on-call rotation as necessary to meet patient care needs. The RN assesses and evaluates patient needs via telephone and advises or makes home visits as required. The RN participates in quality improvement activities and attends in-service training as assigned. The RN performs other duties as assigned by the supervisor.
