Job Opportunity – Registered Nurse Case Manager – Montana

Location: Missoula, Montana

Schedule: Monday through Friday, 8:00 AM to 5:00 PM, with on-call and weekend rotation as part of team scheduling coordination. 8-week contract.
Highlights:
- Provide skilled nursing and case management services to patients in their homes through an interdisciplinary team approach
- Coordinate and integrate patient care services to ensure continuity and quality clinical outcomes
- Manage diverse case mix including venipuncture, catheterization, wound care, and parenteral therapy
- Oversee care provided by Home Health Aides and CNAs to ensure compliance with individualized care plans
- Utilize systematic assessment and OASIS documentation standards for patient evaluations
- Opportunity for career progression from Visiting RN to Case Manager designation
Requirements:
- Current Registered Nurse license in the State of Montana (BSN preferred)
- 2 years of recent acute care, home care, or hospice nursing experience preferred
- Clinical proficiency in venipuncture, catheterization, wound care, and/or parenteral therapy strongly preferred
- Home care documentation experience (HCHB) strongly preferred
- Valid driver's license, appropriate auto insurance, and reliable transportation required
