Must-Have: Proficient in HomeCare HomeBase and OASIS. Recent experience required.
Description:
Provides coordinates and directs the provision of home nursing care according to physician's orders.
Conducts assessments based on agency policies and procedures through the nursing process.
Responsible to the Clinical Manager.
Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses.
May be required to travel for home visits and office meetings.
Must have reliable transportation, a current driver's license, and agency-required liability insurance.
Average number of patients per day: 5-8.
Each type of visit and activity holds a different point value for productivity.
Weekly productivity expectation: 30 points.
Must be willing to travel up to 75 miles from the home clinic.
Mileage will be reimbursed at the IRS standard rate.
All documentation must be completed within 24 hours of the appointment.
Delayed and incomplete documentation that impacts patient care may result in a cancellation.
Must accept every patient scheduled, regardless of distance, declining patients may result in a cancellation.
No guaranteed hours with this contract.
All overtime must be pre-approved.
Travelers must be open to on-call rotations.
Local candidates accepted at the same rate.
Must confirm awareness of the responsibilities involving daily driving to patients' homes.
Comfortable with drives that may exceed 20 minutes between patients.
Must have a vehicle, a driver's license, and auto insurance.
