Hospice Registered Nurse (RN)

We are seeking a dedicated and compassionate Hospice Registered Nurse (RN) to join our interdisciplinary team. This position will involve providing skilled nursing care and support to terminally ill patients and their families in alignment with the patient’s plan of care and hospice philosophy.

Key Responsibilities:

  • Perform comprehensive assessments of patients’ needs and coordinate individualized care plans.
  • Provide pain and symptom management consistent with the hospice plan of care.
  • Educate and support patients and families regarding the disease process, self-care techniques, end-of-life issues, and the hospice philosophy.
  • Collaborate with the interdisciplinary team, including physicians, social workers, chaplains, and volunteers.
  • Maintain accurate and timely documentation in compliance with state, federal, and organizational policies.

Qualifications:

  • Current Registered Nurse (RN) license in the state or compact state RN license.
  • Minimum of 3 years of nursing experience, preferably in hospice, home health, or palliative care.
  • Strong clinical assessment skills.
  • Excellent communication, interpersonal, and organizational skills.
  • Compassionate demeanor with the ability to support patients and families during difficult times.
  • Valid driver’s license and reliable transportation required for home visits.
  • Current BLS Certification.
  • Able to work holiday shifts as necessary.
  • Experience with hospicesoft/careficient EMR is preferred.

Additional Information:

  • Participation in an on-call rotation is required, with availability to respond to patient needs within 45 minutes when on-call.
  • Position requires coverage for weekends and holidays as necessary.
