### Join Our Compassionate Team as a Hospice RN! **Job Description:** Are you a dedicated and compassionate Registered Nurse (RN) looking to make a profound impact on the lives of terminally ill patients and their families? If so, we want you to join our interdisciplinary team in providing high-quality, end-of-life care within the heart of the stunning Moab, Utah! **Key Responsibilities:** - Perform comprehensive assessments of patients' needs. - Coordinate individualized care plans tailored to each patient. - Provide effective pain and symptom management. - Educate patients and families on the disease process and self-care techniques. - Collaborate with a team of healthcare professionals, including physicians and social workers. - Participate in an on-call rotation to ensure timely patient support. - Maintain accurate and timely documentation in compliance with policies. **Qualifications:** - Current Registered Nurse (RN) license in Utah or compact state. - Minimum of 3 years of nursing experience, preferably in hospice, home health, or palliative care. - Strong clinical assessment skills. - Excellent communication, interpersonal, and organizational skills. - Compassionate demeanor to support patients and families in difficult times. - Valid driver’s license and reliable transportation required. - Current BLS Certification. - On-call availability for 1-3 shifts per week and ability to respond within 45 minutes when on call. - Experience with Hospicesoft/Careficient EMR preferred. **Spectacular Location:** Experience the breathtaking beauty of Moab, Utah!
