Job Title: Hospice RN Profession: Registered Nurse Specialty: Hospice Duration: 13 weeks Shift: 12-hour Day Shift Hours per Shift: 36 Experience: Minimum 3 years of Hospice experience (home or inpatient) License: Current RN license Certifications: BLS, Driver's License, personal vehicle, car insurance Must-Have: - Minimum 3 years of Hospice experience - BLS certification - Driver's License and personal vehicle - HCHB EMR experience Description: Provide hospice homecare to patients in assigned areas. The current census for the department is 120-130 patients. Average caseloads are 12-15 patients, with typically 5 visits per day. All adults are the focus, primarily geriatrics. Common diagnoses include end-of-life conditions such as CHF, COPD, respiratory failure, kidney failure, liver failure, Alzheimer's, dementia, and oncology patients. Critical thinking and strong assessment skills are required. The RN will manage all aspects of the hospice experience autonomously. General wound care and management of opioid/narcotic medication education are essential skills. Several standing orders are in place, reducing the need to call physicians for every order. Home care aides work independently of RNs, and support is available from a Triage RN for unexpected needs and patient call-ins. Technology used includes EMR: HCHB (preferred) and Symplr. The schedule will involve weekend rotations with open shifts available. A commitment to covering 4 hours of assigned holidays may be required. The unit culture promotes excellent teamwork and cohesiveness.
