Home Care Registered Nurse

Job Description:

Responsible for providing high-quality nursing care and hospice coverage when on-call. This role requires effective communication, thorough assessment skills, and strong case management capabilities to ensure optimal patient outcomes.

Responsibilities:

• Complete OASIS documentation accurately and timely, adhering to agency policies, state, and federal regulations.
• Utilize the nursing process to assess the needs of patients and caregivers, ensuring care is based on evidence-based practice.
• Develop and communicate realistic care plans in collaboration with patients and caregivers, following appropriate regulations and guidelines.
• Act as Case Manager for assigned patients, managing caseloads and ensuring compliance with agency productivity standards.
• Implement medical plans of care and nursing interventions effectively to guarantee high-quality patient care.
• Maintain proficiency in high-tech nursing skills, including IV use and complex wound care, ensuring infection control standards are met.
• Collaborate with LPNs to ensure adherence to care plans, providing guidance and delegating tasks appropriately.
• Evaluate the effectiveness of care plans, considering identified problems and patient responses to ensure overall quality of care.
• Document all nursing care accurately to facilitate continuous quality improvement and care integration.
• Follow established protocols for documentation and patient visits using McKesson processes.
• Educate patients and caregivers to support informed decisions regarding their care, covering medications, treatment options, and discharge instructions.
• Identify interdisciplinary needs and communicate them effectively to relevant team members, including physical therapy and social services.
• Complete necessary agency, state, and federal forms accurately and in a timely manner.
• Conduct patient visits efficiently, ensuring supervisory visits are appropriately scheduled.
• Assess patients' needs for Home Care Aide services and coordinate ongoing supervision as needed.
• Report patient incidents to Quality Improvement Specialists promptly.
• Ensure safe patient discharges and transfers with all paperwork processed according to policies and regulations.
• Coordinate with healthcare providers and team members to support the plan of care.
• Participate in monthly peer review audits, providing constructive feedback and accepting it professionally.
• Proactively schedule and facilitate case conferences to enhance communication amongst team members.
• Advocate for patient care issues to ensure care is tailored to the unique needs of each patient and their family.

Shift Information:

Monday to Friday, 8 AM to 4 PM, with some weekends and holiday coverage required.

Qualifications:

• Minimum of 2 years of nursing experience required.
