Job Description & Requirements

REQUIRED EXPERIENCE

• Home Health and Hospice nursing experience required
• Experience with OASIS documentation and home health assessments
• Experience developing and managing individualized plans of care
• Case management and patient visit coordination experience
• High-tech nursing skills, including:
  • IV use and maintenance
  • Wound VAC
  • Complex wound care
• Experience with patient/caregiver education, discharge planning, and care coordination
• Experience supervising LPNs and Home Health Aides within scope of practice
• Strong documentation, communication, and interdisciplinary collaboration skills
• Knowledge of Medicare, state/federal regulations, and home health policies

SCHEDULE

• Sunday–Thursday
• 8:00 AM–4:00 PM

LICENSE

• Vermont nursing license required
