Hybrid in-home + on-site position - Home Health Psych

Years of experience: 1 year of any psych

Will you accept a first-time traveler? No

Required License, National Certification, Certs: BLS, personal vehicle, driver’s license, & auto insurance; CPI or MOAB highly preferred

Patient Types/Common Diagnoses: adults only (avg span of time in program is @ 4 years);Schizophrenia (stable/outpatient management) & Schizoaffective Disorder are primary diagnoses; secondary can = Bipolar I Disorder, Bipolar II Disorder, Major Depressive Disorder (MDD), Persistent Depressive Disorder (Dysthymia), Anxiety & Trauma Related Disorders, Generalized Anxiety Disorder (GAD), Post Traumatic Stress Disorder (PTSD), Obsessive Compulsive Disorder, Attention & Neurodevelopmental Disorders, Attention Deficit/Hyperactivity Disorder (ADHD), Psychotic Disorders (Stable), Delusional Disorder, Psychosis, unspecified (when stable), Borderline Personality Disorder, Avoidant Personality Disorder, Obsessive Compulsive Personality Disorder

Patient Ratios: case load is @10 pts, but will also address various medical needs of anyone in program

Skills required: Injections (long acting anti-psychotics), refill coordination, Prior Authorizations, Outlook, psychotropic medication management, side effect assessment (EPS, tardive dyskinesia, metabolic concerns, etc.), AIMS assessments, physical health screening and monitoring, crisis assessment, including suicide and violence risk evaluation, substance use assessment and intervention, independent decision-making in the field, safety awareness and risk assessment, home visit and outreach skills, engagement of difficult-to-reach clients, flexibility and adaptability in non-traditional care environments, resource navigation and community collaboration

Unit specific equipment: laptop & cell phone provided

Floating: this is a position for within the community to patient’s houses, as well as working on site at the Health and Wellness Center prn
