- Requirements:
- Minimum of 3 years of experience in performing OASIS assessments and care planning for in-home patients.
- Previous experience with Epic software is a plus.
- Shift Information:
- Enjoy a day shift schedule (Monday through Friday) while also being part of the call rotation.
- Participate in a one in four call rotation for nights, weekends, and holidays.
- Flexible work schedule with 32-40 hours per week available.
