Job Title: Registered Nurse (RN) Profession: Nursing Specialty: Home Health Duration: 260 Days Shift: Day Hours per Shift: 12 Experience: Minimum of two (2) years in a home health setting within the last fifteen (15) years. License: Current RN Licensure in Client Certifications: Basic Life Support (BLS) required before start date Must-Have: - Recent experience conducting comprehensive assessments of patients. - Two (2) years of experience in a home health setting. - Experience working in a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care. - Experience conducting assessments similar to UAS such as OASIS. - Providing primary care to homebound individuals and managing chronic diseases. Description: This position requires comprehensive evaluation of patient health status. Nurses will perform initial in-home pre-evaluations. The role involves evaluating health needs, strengths, and preferences of patients. Nurses guide the development of individualized long-term care service plans. Documentation of consumer medical records and health referrals is required. Evaluation data must be entered into electronic forms and transmitted. Maintaining continuity of care by reducing gaps in treatment plans is essential. Discussion of healthcare options and necessary supports with consumers is expected. Initiating communication for enrollment into managed care health plans is part of the responsibilities. Collaboration with interdisciplinary care team members is crucial for care coordination. Quality review data and documentation for outcomes must be collected. Opportunities for health promotion and illness prevention should be identified. A comprehensive working knowledge of community resources is needed. Meeting all established performance standards is mandatory. Additional duties may be assigned by management as necessary.
