Must-Have:
- Two (2) years of experience in a home health setting within the last ten (10) years, which may include working in the field for a Certified Home Health Agency, Licensed Home Care Service Agency, or Managed Long-Term Care.
- Conducting assessments that are similar to the UAS, such as OASIS.
- Providing primary care to homebound individuals.
- Care management for individuals receiving home and community-based services, including hospice and assisted living.
- Home care experience as a paraprofessional or Licensed Practical Nurse caring for a minimum of two patients per week.

Nurses must have recent experience conducting comprehensive assessments of patients.
All nurse assessors must meet specific requirements, including two years of experience in a home setting within the last ten years.
This may include working in the field for various home health agencies or conducting assessments similar to UAS or OASIS.
Preferred qualifications include experience conducting UAS assessments in the last 365 days.
Candidates should have experience caring for a geriatric population and managing chronic diseases in the home setting.
A combination of home care experience as a paraprofessional or LPN in conjunction with RN duties is highly regarded.
Responsibilities include performing initial in-home evaluations of patients, reviewing medical documentation, and entering evaluation data into electronic forms.
Candidates must document any concerns or conflicts that arise during evaluations and emphasize continuity of care to prevent fragmentation.
Communication regarding healthcare options and service support with consumers is essential.
Interdisciplinary communication is necessary to improve care coordination.
Candidates must collect quality review data and maintain a working knowledge of community resources and payor requirements.
Meeting performance standards established for this position is required.
