Registered Nurse - Geriatric Home Health & Clinic

Job Summary

We are seeking a dedicated and experienced Registered Nurse to provide comprehensive clinical care to our geriatric patient population in Kalamazoo, Michigan. This role involves a dynamic blend of clinic-based and home-based direct nursing services, focusing on the unique needs of frail and elderly participants.

The Clinical Care Registered Nurse operates with a high degree of independence, applying advanced nursing knowledge and skills to manage complex clinical situations. You will work collaboratively with Primary Care Providers (PCPs) and an interdisciplinary team to ensure coordinated and holistic patient care.

Key responsibilities include triaging emergent situations, monitoring acute and chronic medical conditions, and executing individualized care plans. This position is vital in supporting the health and well-being of our participants, both within our facility and in their homes.

Key Responsibilities
  • Triage emergent situations and apply advanced nursing knowledge independently.
  • Monitor acute and chronic medical conditions in close coordination with Primary Care Providers (PCPs).
  • Independently utilize and implement established care plans to provide direct nursing care in both clinic and home settings.
  • Administer treatments, procedures, and medications as prescribed to participants.
  • Conduct regular home visits to provide direct care, which may include wound care, lab draws, comprehensive nurse assessments, and other essential nursing tasks.
Required Qualifications
  • Associate's Degree in Nursing from an accredited School of Nursing or University.
  • Minimum of one (1) year of experience working with a frail, elderly, or long-term care population.
  • Current Basic Life Support (BLS) certification from the American Heart Association (AHA).
  • Must possess a current, valid Michigan driver’s license, reliable transportation, and active car insurance.
  • Must be 18 years of age or older.
  • Ability to comply with vaccination requirements for communicable diseases including Influenza, MMR, Varicella, Tdap/Td, Covid-19, and Hepatitis B.
Preferred Qualifications
  • Bachelor’s Degree in Nursing (BSN).
  • Minimum of one (1) year of home care experience.
  • Minimum of one (1) year of wound care experience.
Skills & Competencies
  • Advanced clinical judgment and independent decision-making in complex nursing situations.
  • Proficiency in triaging emergent medical conditions.
  • Strong ability to monitor and manage acute and chronic medical conditions.
  • Expertise in developing, implementing, and evaluating individualized care plans.
  • Skilled in medication administration, various treatments, and clinical procedures.
  • Competency in performing wound care, phlebotomy (lab draws), and comprehensive nurse assessments.
  • Excellent verbal and written communication skills for effective interaction with participants, caregivers, staff, and external organizations.
  • Strong collaborative skills to work effectively within an interdisciplinary team and with PCPs.
