Job Summary
Join our dedicated team as a Registered Nurse specializing in Geriatric Clinical Care, providing essential services to the frail and elderly population across Battle Creek and Albion, Michigan. This float role offers the opportunity to deliver direct, independent nursing care within both clinic and home-based settings, managing complex clinical situations and collaborating effectively with a multidisciplinary team.
The successful candidate will leverage advanced nursing knowledge and skills to monitor acute and chronic medical conditions, implement comprehensive care plans, and administer a wide range of treatments, procedures, and medications. This position is vital for ensuring high-quality, compassionate care for our participants, requiring strong clinical judgment and a commitment to independent practice.
Key Responsibilities
- Provide direct nursing care to participants in both clinic and home-based environments.
- Independently triage emergent situations and apply advanced nursing knowledge and skills.
- Monitor acute and chronic medical conditions in close coordination with Primary Care Providers (PCPs).
- Develop and utilize individualized care plans to guide participant care.
- Administer prescribed treatments, procedures, and medications accurately and safely.
- Conduct regular home visits to perform wound care, lab draws, comprehensive nurse assessments, and other essential nursing tasks.
- Collaborate effectively with licensed and non-licensed personnel to optimize participant outcomes.
Required Qualifications
- Associate's Degree in Nursing from an accredited institution.
- Minimum of one (1) year of experience working with the frail, elderly, or long-term care population.
- Current Basic Life Support (BLS) certification from the American Heart Association (AHA) or ability to obtain prior to start.
- Valid Michigan Driver's License, reliable transportation, and current automobile insurance.
- Ability to comply with all organizational vaccination requirements, including but not limited to Influenza, MMR, Varicella, Tdap/Td, Covid-19, and Hepatitis B.
Preferred Qualifications
- Bachelor of Science in Nursing (BSN).
- Minimum of one (1) year of home care experience.
- Minimum of one (1) year of wound care experience.

- Advanced clinical assessment and critical thinking skills.
- Proficiency in triaging emergent situations and managing complex clinical cases.
- Strong ability to work independently and make autonomous clinical decisions.
- Excellent verbal and written communication skills for interacting with participants, caregivers, and interdisciplinary team members.
