Description: This position requires a registered nurse specializing in case management. The role involves actively participating in discharge planning and care coordination. You will organize and conduct family meetings to discuss treatment plans. Experience with various patient demographics including neonates, adolescents, adults, and geriatrics is necessary. The daily patient census is approximately 25. You will work closely with interdisciplinary support teams including physical therapy and social services.
