Must-Have:
- Experience in assessing patients and care coordination.
- Knowledge of admission criteria, benefits eligibility, and discharge planning.
- Competency in arranging durable medical equipment, skilled nursing facility placement, and ambulance transport.
- Familiarity with family meetings and treatment plan development.
- Understanding of advance directives and protective services processes.
- Ability to work with various patient populations, including neonates, adolescents, adults, and geriatrics.
Description:
This position is for a registered nurse specializing in case management.
The nurse will participate in care coordination and utilization review processes.
Experience with HMOs and IPAs is preferred.
The role requires a commitment to working every other weekend to meet department needs.
Patient ratios will be 1:25 during the day and 1:45 on weekends.
The nurse will collaborate with interdisciplinary support staff, including physical therapy and social services.
Daily census may vary based on patient needs.
Travelers may be expected to float between nearby facilities as assigned.
Refusal to float may result in assignment termination.
Candidates must respond to questions regarding previous employment at any associated facilities.
References covering the past year are required.
Communication devices will be provided as needed to perform duties.
The nursing staff is expected to manage their time effectively, including completing required modules.
