**Job Responsibilities:**
- Manage patient care plans and coordinate with medical teams for optimal recovery.
- Work with a diverse patient population in an Intermediate Rehabilitation Facility (IRF).
- Collaborate effectively with healthcare professionals to ensure comprehensive care.
- Maintain accurate documentation using EPIC EMR.
**Required Skills/Experience:**
- Must possess a valid RN license.
- Minimum of 1 year experience in Case Management.
- Preferred experience with EPIC EMR.
- Must be a collaborative and flexible team player.
**Unit Information:**
- Total Beds: 26
- Patient Ratios: 1/2 census case load
