Duties :
- Coordinates patients through the acute care hospitalization Manages all aspects of discharge planning/transition process Performs utilization management and quality screenings In- Patient Case Management is required .

Skills :
-Hospital experience required, including experience with discharge planning and placement in rehabilitation facilities Minimum 2 years of experience with acute inpatient hospital discharge planning Rotating weekends: Every 4th or 5th weekend Holidays: One summer and one winter holiday Experience with subacute patients required
- 2 years of acute care experience strongly preferred
- Case Management, Managed Care. In Patient Case Management is required

Education :
- BSN required
- RN license required
- CCMC preferred
