Location: Eureka, CA
Shift: Days, 5x8 — 8:00 AM–4:30 PM
⏰ Guaranteed Hours: 40 hrs/week
Call: None
Weekends: Every other weekend
EMR: Epic
Float: Yes, based on department/sister-facility needs
Local: Accepted at lower bill rate

Job Details

• Acute-care hospital Case Management
• Caseload up to 25 patients
• Patient population: Adolescents, adults, pediatrics & geriatrics
• Interdisciplinary support with Social Services
• Must have current acute-care hospital case management experience within the last year
• Travel experience required
• First-time travelers may be considered with clinical approval

Required Experience/Skills

• 2+ years of experience
• Acute hospital case management
• Epic experience
• HMO/IPA experience
• Advance directives & POLST
• Protective services process
• Admission criteria
• Discharge planning & care coordination
• Prior authorizations
• Appeals & denials

• DME needs assessment
• CMS / DRG / Core Measures knowledge
• Workers' compensation case management

Important

• Compact RN license must be in hand at submission
• 50-mile local radius
• Must work 3 of 5 holidays
• All RTO must be submitted upfront
• 2 references covering 1 year within the last 3 years
• Must answer whether they previously worked at a Providence facility
