The role involves supporting patients with Huntington’s Disease and Multiple Sclerosis. Responsibilities include contacting patients and screening for Huntington’s Disease. The social worker will assist in assessing socioeconomic constraints. Support will be provided with disability forms and scheduling appointments. Training and education will be available for those new to Huntington’s Disease. This position is part-time, requiring availability every Monday and two additional days each week. A background in managing complex patient care is preferred. Experience with behavioral and mental health needs is beneficial. Knowledge of Department of Children and Families processes is a plus. Candidates should have experience in biopsychosocial assessments. This is an outpatient role with no call or weekend requirements.
