Description

American Traveler is hiring a Home Health RN with HCHB proficiency and OASIS experience for a Day shift assignment in home health settings.

Details

• Home Health unit providing adult home health services
• Day shift schedule, 5x8 hours from 8:00 AM to 4:30 PM
• Each traveler assigned to their own territory
• On-call rotation may be required
• Personal transportation and valid driver's license required

Requirements

• Active OR RN license required in-hand at time of consideration
• BLS certification required
• Minimum of 1 year of relevant home health experience required
• OASIS experience required for staff completing Start of Care (SOC)
• Proficiency in HomeCare HomeBase (HCHB) charting system required
• Experience with wound care, medication administration/management, and pain assessment
• Skilled in IV starts, phlebotomy, and infection control precautions
• Experience with clinical assessment documentation and obtaining vital signs
• Prior home health travel experience preferred, though not strictly required

Additional Information

• Responsibilities include admission assessments, discharge planning, recertification, resumption of care, and routine visits
• Completes documentation for wound care, medication management, and pain assessment
• Performs medication administration via intramuscular, intravenous, and subcutaneous routes
• Monday orientation required, including completion of billable training modules
• First-time travelers will not be considered for this position
• Former permanent employees must be separated for at least 6 months and approved by HR before returning as a traveler
• Travel pairs accepted, provided each traveler maintains their own territory and transportation
• RTO requests must be submitted at the time of application
