**Job Description:**
- **Shift:** Day 5x8-Hour (08:00 - 17:00)
- **Experience Required:** Minimum of 3 years in acute care case management
- **License Required:** Active California RN License
- **Certifications:** BLS
- **Flu Vaccine:** Required (no exemptions)
- **Special Requirements:** EPIC experience highly preferred

**Qualifications:**
- **Patient Care Experience:**
- Charge Experience: Not required
- Patient Ratio Experience: Varied
- Charting System Experience: Preferred (Epic)
- **Experience with Community Hospitals and LTAC:** Preferred
- **Skills:**
- Acute Hospital care coordination
- Familiarity with CMS, CPT coding, DRG, ICD 10 Coding, NCQA, OSHA guidelines
- Expertise in Discharge Planning and Plan of Care
- Utilize InterQual and Milliman Guidelines
**Unit Details:**
- **Patient Age Groups:** Adolescents, Adults, Geriatrics
- **Daily Census:** 255
- **Number of Beds:** 14
- **Interdisciplinary Support:** Includes Social Services and Transportation
**Submission Rules:**
- Valid California license must be in hand at submission
- 2 references from the past year required (supervisor level)
- Previous employees must have 24 months of separation before eligibility for travel positions
**RTO Rules:**
- Max RTO: 2 weeks total
- No RTO within the first 2 weeks of the assignment
