We are seeking an organized and patient-focused Patient Care Coordinator to join an established healthcare team in Charlotte, NC. This position plays an important role in ensuring physician orders, referrals, appointments, authorizations, and medical records are completed accurately and on time.

Key Responsibilities

Schedule patient follow-up visits, ancillary appointments, procedures, and referrals
Follow up on referrals and appointments to ensure physician orders are completed in a timely manner
Complete forms and requisitions necessary to carry out physician orders
Obtain and send medical records to physicians and outside facilities
Obtain laboratory, X-ray, hospital, referral, and other ancillary results
Verify insurance coverage and obtain/document authorization requirements for referrals and procedures
Monitor assigned Athena/EHR inboxes and ensure documentation is handled appropriately
Assist patients with questions regarding appointments, referrals, and access to care
Maintain accurate and complete patient charts and documentation
Communicate with physicians and healthcare professionals as needed
Work at different office locations as needed

Qualifications

High school diploma required; some college preferred
Minimum 1 year of medical office or healthcare facility experience
EHR and scheduling experience required
Experience providing customer/patient service in person and over the phone
Strong organizational, communication, and attention-to-detail skills

Health Requirements

Completion of a TB test required
Hepatitis B and flu vaccination documentation or applicable declination forms required
