About the Opportunity

Join a growing home healthcare organization dedicated to helping seniors remain independent, safe, and comfortable in their homes. This highly visible role serves as a key liaison between referral sources, clients, families, healthcare providers, and internal care teams. The Healthcare Intake Coordinator is responsible for coordinating referrals, admissions, insurance verification, authorizations, and client onboarding activities to ensure a seamless care experience.

Key Responsibilities

• Coordinate incoming referrals and admissions
• Review referrals and gather necessary documentation for client onboarding
• Verify insurance eligibility and obtain required authorizations
• Process medical orders, referrals, and related clinical documentation
• Enter and maintain client demographics, medical information, medications, and documentation in the EMR system
• Serve as a liaison between referral sources, clients, families, providers, and internal care teams
• Create and maintain client care plans and required records
• Track referral activity and client information using spreadsheets and internal systems
• Respond to client questions, concerns, and service requests in a professional and timely manner
• Assist with scheduling activities and occasional on-call support as needed
• Maintain accurate documentation and ensure compliance with company and regulatory requirements

Qualifications

• Associate degree required; Bachelor's degree preferred
• Experience in healthcare intake, admissions, referral coordination, patient access, or healthcare administration
• Medicare intake experience preferred
• Home health, hospice, or home care experience preferred
• Knowledge of insurance verification, prior authorizations, and payor types
• Familiarity with medical terminology
• EMR/EHR experience required
• Strong organizational, communication, and customer service skills
• Certified Medical Assistant (CMA) preferred

• Location: Woodbury, MN (100% Onsite)
• Schedule: Monday-Friday | Standard Business Hours
• Employment Type: Temp through the end of the year
• Compensation: $26-$29/hour
